What is the procedure of getting Duplicate Certificate?

For getting duplicate certificate the following procedure may be carried out:

1. The candidate/applicant should fill the application form.
2. Paste his fresh photograph on the form.
3. Get this form attested from the Head of the Institution that is affiliated with BISE Multan.
4. Attach CNIC attested copy with the form.
5. Press cutting in any National Newspaper regarding lost/theft of the certificate and attached the newspaper with the application form.
